How to sew a quilt with padded detail?

Hi, I'm not quite sure how to put this but I saw a beautiful quilt using a piece of fabric with little animals on it and they had somehow inserted padding underneath some of the animals they wanted to feature so the majority of the quilt was flat per say but the highlighted animals looked puffy so they stood out? Sorry, I'm very new at this! Thanks :)
